Children’s Health Insurance Program (CHIP)
The CHIP program offers free or minimal cost coverage to eligible children for doctors’ and hospital visits, vision and dental care. The Clay-Battelle Health Services Association is a participating provider and can help you complete eligibility applications.
Medicaid
Medicaid is a program available to those who meet certain income or medical guidelines. Our locations participate in most Pennsylvania and West Virginia Medicaid plans and can help you complete an application.
Enrollment Services
The Clay-Battelle Health Services Association employs a full-time Certified Application Counselor to assist patients in finding affordable health coverage and other help through many different programs. Mileage Reimbursement Program
Uninsured patients of the Clay-Battelle Health Services Association may be able to receive mileage reimbursements for travel to and from our locations. To be eligible for reimbursements, you will need to sign the Mileage Reimbursement Program Policy. If you have questions, please call us at (304) 432-8211 in Blacksville or (304) 775-4671 in Burton.
Reproductive Health Program
The Clay-Battelle Health Services Association offers confidential reproductive health services including pregnancy testing; breast, cervical, and testicular cancer screenings; PAP tests; testing and treatment for some STDs; and several forms of free contraceptives to residents of Pennsylvania and West Virginia.
Breast and Cervical Cancer Screening Program
The West Virginia Breast and Cervical Cancer Screening Program offers free or low-cost mammograms and pap tests for women with limited income in West Virginia. You may be eligible even if you have insurance.
Vaccines for Children Program
The Vaccine for Children Program is available to eligible children in West Virginia and Pennsylvania and provides free vaccines for required immunizations.
The Health Insurance Marketplace
The Marketplace is a new location to find individual and small business health insurance coverage. Premium and cost-sharing reductions may be available. The Clay-Battelle Health Services Association can assist you in comparing coverage and completing applications.
Sliding Fee Discounts
We provide sliding fee discounts for patients based on their income to help with out-of-pocket expenses. Sliding fee discounts are available on both medical and dental services.
About our Sliding Fee Discounts
The Clay-Battelle Health Services Association offers a Sliding Fee Discount program to help with out-of-pocket expenses for all patients based on household size and income. This discount program allows you and your family to pay a reduced fee for covered medical and dental services at both of our locations. There are 4 discount levels available. You can see the discount levels on this page.
In order to qualify for our sliding fee scale, you will need to complete a Sliding Fee Application and provide proof of income. Acceptable proof of income would include your tax return, W-2, recent pay stubs, Social Security benefits statement, or other documentation.
